Our vision at LDK is to help you to live independently, in your own home, for your entire life. Our One Move Promise® is that when you call an LDK Seniors’ Living™ village home, you will never need to move again.

As part of our One Move Promise®, we’re often asked how we plan to support our residents through to the end of their life. To help answer that, we’ve broken down what care at LDK looks like.

The support that we provide is delivered primarily through what we call ‘planned care’. This involves us meeting with you to undertake a care assessment and understand your unique circumstances, goals and preferences. Our Care Team partners with residents to create their own individual care plan to ensure the proposed care and services meets your needs and circumstances. Once agreed, care is then delivered in the comfort and privacy of your own home. Your care plan is regularly reviewed by our clinical team to account for care and lifestyle needs that may change down the track.

24/7 Care

Our onsite Care Team of Personal Carers are all supervised by our onsite Registered Nurses. We are committed to ensuring the safety of our residents so our onsite care team is available 24 hours a day, 7 days a week. We also have Case Managers, who assists residents with accessing government funding which you may be entitled to access.

Whilst our planned care is mostly determined in advance, we know circumstances can change without notice. Our onsite care team is available and trained to respond to any unexpected or emergency events – giving residents peace of mind, knowing we’re there if something happens.

Dementia Care

Our LDK Seniors’ Living™ villages are designed to care for our residents at any stage in the ageing journey. So, rest assured if you or a loved one require dementia care, it is available onsite. Care for our residents with cognitive needs can include tailored daily routines and specialised therapy as part of their planned care.

Respite Care

Respite care provides a little extra support or care for a short period. It’s designed to benefit partners or primary caregivers who may need a break. Our warm and welcoming respite care is available on a case-by-case, as-needed basis.

End of Life Care

When our residents enter the last stages of their ageing journey, we look after them with Love, Decency and Kindness and provide increased care and support in the comfort of their own LDK home, where their loved ones, friends, family and pets can be by their side.
